Transportation for the Vaccine for Students

If you signed up your child (16 and older) to take the vaccine and requested transportation here, the bus will leave Fordson at 11am.  The bus will be here on April 8th, 9th, and 16th.  

Students must have the following with them:

  • The transportation permission slip 
  • DMC consent form 
  • DMC Outpatient consent form 
  • Copy of driver’s license or school ID if you do not have a driver’s license
  • Copy of insurance card
